Pembrokeshire County Council: The Managed Care Team and the Review Team

Introduction

The Managed Care Team and the Review Team work with those who at 18+ with physical disabilities, learning difficulties, older adults and those with mental health difficulties. This is always to ascertain their wishes and feelings on what they may like to change and how they can achieve their goals as per the Social Services and Wellbeing Act 2014. As part of the ongoing digital development, we have considered how best to support our team’s need for ongoing professional development alongside the tools needed to support us on a daily basis. In light of our new ways of working providing ongoing challenges for us all in accessing the information we need to support us in our day to day business we have developed an easy-access system to enable you to access the latest guidance and most up to date information tailored to support the needs of the team.
